·         I agree to show up for the appointment time I schedule with the mechanic.  If I am not able to get there at the agreed time, I agree to contact the mechanic within 24 hours to explain the situation and reschedule another appointment.

·         I agree to arrange childcare for my child(ren) for the time my car is being worked on.  I understand this is so I can give the mechanic my undivided attention, to watch and learn about my car and how to fix things myself.

·         If the mechanic does temporary work on my car and recommends that other work be done as I can afford it, I agree to follow up on this with my PS-S Advisor.  The reason is that other, more costly, damage to my car can be prevented by keeping up with the suggested maintenance.

·         I understand that this appointment for car repair is for one time only.  If I need future car repairs, I agree to notify my Advisor rather that calling the mechanic directly.  This is done to spread the jobs among the different mechanics through scheduling.


·         I agree to take this form and the Vehicle Repair Request Form with me to my appointment with the mechanic

STAGE AND SOUND EQUIPMENT
In order to assure a quality concert, PURCHASER agrees to provide ARTIST
with the following, at PURCHASER'S sole cost and expense:

            A.  A stage for performance which must be a minimum of  two hundred and twenty  (220) square feet.  The stage must be flat, smooth and stable.  A close cropped gray carpet measuring six (6) feet by (6) feet for the drum kit.

            B.  A professional lighting system suitable to adequately illuminate the stage area.

            C.  A professional sound augmentation system, with at least 24
inputs, capable of providing clear, undistorted, evenly distributed sound throughout the audience area.  The system must be complete with at least fifteen (15) high quality low impedance microphones.  Also five (5) high quality monitor speakers (see stage diagram attached).  An engineer to operate said system for the full sound check and performance.

The Grantee shall be responsible for the performance of the work as set forth herein below and for the preparation of products and reports as specified in this Agreement.  The Grantee's Project Representative shall promptly notify the State of events or proposed changes that could affect the Work Plan under this Agreement.

Special Provisions

1.      Grantee shall complete all work in accordance with an approved Work Plan which will be included in this Agreement as Exhibit A “Tasklist and Timeline”, Exhibit B “Budget,” and Exhibit C “Monitoring and Assessment Plan.”

2.      If the Project includes acquisition of real property:
a.       As conditions precedent to the State’s obligation to deposit the Grant Amount in escrow, the Grantee shall submit to the State an appraisal for review and approval in writing by the Department of General Services (DGS) of all documents pertaining to the Real Property appraisals and property transaction, and provided copies to the State.  The Grantee shall be required to establish a billing account with the Department of General Services for this purpose.

b.      A Memorandum of Unrecorded Grant Agreement, Exhibit D must be recorded by the Grantee to provide notice of this Agreement between the Grantee and the San Gabriel and Lower Los Angeles Rivers and Mountains Conservancy.

3.      If the Project includes development, landscaping and/or planting:

a.       The Grantee shall include a representative of the State on the selection panel for contracted services.


b.      The Grantee shall submit a Project Development Plan, Exhibit E, in accordance with RMC General Policies, Exhibit F, to the State for approval prior to the solicitation of a subcontractor or awarding of a bid for services.

Example 1
The use by the Company of the Sponsor’s logo, trademarks, and tradename under the terms and conditions of this Agreement shall inure solely and exclusively to Sponsor, and the Company shall acquire no goodwill or other interest in them.  Sponsor hereby grants to the Company the restricted license to use the Sponsor’s name for purposes of identification and the promotion of specified events and activities of the Company.   Sponsor also hereby grants to the Company the restricted license to use the Sponsor’s logo associated with the name of the Sponsor to carry out the Company’s obligations under this Agreement.  The term “restricted license” hereby requires Company to obtain prior written approval by the Sponsor on any and all items for which the Sponsor’s logo, trademark, and tradename will be used.  This includes, but is not limited to, banners, promotional items, advertisements, and the like.

Example 2

  1. You may not frame, post, upload, transmit or in any way use the ESSA Logo/s or ESSA Intellectual Property, unless it is provided for in these Terms and Conditions or it is expressly authorised in writing by ESSA.
  2. You may not modify the ESSA Logo/s in any way.
5. Permitted purpose
  1. The ESSA Intellectual Property must be used solely for the purpose of:
    1. Promoting that you are a current and financial member of ESSA; (ESSA logo mark).
    2. Promoting that you are a current and financial accredited member of ESSA (Accreditation logo marks)
    Example 3
     Logo will be provided by Chamber in electronic format.  Logo may be used in black and white or color.  If color log, Member agrees to follow correct PMS color scheme (Coated PMS Gold 1255 & Blue 295 or Uncoated Gold 7405 & Blue 287 - Post 2001 Pantone Book only).  Logo must be kept at the ratio 75% of logo width for the high.  (Example: if logo is 2 inches wide then it could be 1.5 inches high.  Take the desired width size and multiply by .75 to get the correct height.)

    3.  Chamber reserves the right to approve all uses of its name and logo.  Member must provide detailed information of how logo will be used.  If used in a print advertisement, Member must provide name and date(s) of publication.  If used in a broadcast advertisement, Member must provide station call letters and flight dates of ad.  If used in a promotional advertisement, Member must provide date of promotional event and details of promotion's purpose. For website usage linking instructions will be emailed.

    4.  Member must provide Chamber with a copy of advertisement BEFORE scheduled publication or broadcast.

    5.   Primary Contact for all logo requests will be the Networking & Communications Coordinator.  A Member's use of the Chamber logo must be jointly approved by the Marketing & Communications Director and Networking & Communications Coordinator.  Upon approval, the Networking & Communications Coordinator will forward the electronic files and proper logo guidelines to the Chamber member.

    6.  Chamber reserves the right to reject requests for use of Chamber logo, name, or likeness.

    7.  All use of Chamber logo, name, and likeness must cease and desist upon non-renewal of membership.

    8.  Company acknowledges that the name and logo are the sole and separate property of the York County Chamber and any use by Company hereunder shall not give rise to any right of use or ownership except as set forth herein.
